Before your scheduled meeting with a Benefit Counselor, please ensure you have the following information readily available for yourself and your dependents:
Social Security Number (SSN)
Date of Birth (DOB)
Full Legal Name
Additionally, you will need a method of payment to complete your enrollment. Accepted forms of payment include:
Debit Card
Credit Card
Bank Account and Routing Number
